Ритуалът Сурва е запазил своята автентичност през вековете и продължава да бъде жив израз на българския фолклор, музиката и народното творчество.
Съчетаването на страховитото и комичното е важен елемент. Характерна част от облеклото са маските, които изобразяват различни митични същества, животни или фантастични образи, а костюмите често съдържат хумористични препратки към злободневни теми. Звънците се свързват със силата на звука да прогонва злото и да привлича добрите енергии. Легенди, приказки и вярвания разказват, че от звънчевата песен бягат всички дяволи, караконджули, таласъми и вампири.
В нощта на ритуала сурвакарите обикалят цялото село, като изпълняват специални танци и обредни действия на открито. Те посещават домовете на хората, пожелават им здраве, плодородие и берекет, а в замяна стопаните ги даряват с храна, вино или дребни пари. Ритуалът включва и запалването на големи огньове на високи места в селото, като около тях се играят ритуални танци. Огънят се счита за пречистващ елемент, който унищожава всичко лошо и носи светлина и топлина за новата година.
Огънят, прътът и водата имат особено значение в кукерските празници. В традиционния народен календар периодът от Коледа до Водици, когато играят сурвакарите, е познат като “мръсни дни”. Вярва се, че тогава бродят вредоносни нечисти сили, които изчезват едва след като на Водици се “кръсти” водата. Маскиранията са глобален културен феномен, те не са затворени в границите само на една етническа традиция и затова винаги са представлявали интерес едновременно и в национален, и в международен мащаб.
